We are currently looking a Waking Night Support Worker to join our team at our small residential service in South Brent.

This is 1 night a week on every Monday.

It has good links to surrounding areas such as Totnes, Plymouth, Ivybridge and Exeter with a 3-minute walk to the local bus stop. The ladies that access the service enjoy accessing the community, this is enabled by two vehicles being at the property.

As a Support Worker you will join a dedicated team working with 4 ladies in their home. You will support the individual to maintain dignity through personal care

The successful applicant(s) will be working the following Rota:
Week 1:
Monday - Waking night: 21:00 – 07:00
Week 2:
Monday - Waking night: 21:00 – 07:00

You will work with the Practice Leader to lead a team providing a combination of delivering 1:1 support and core background support within a shared hub for 6/7 individuals living in their own flats with access to a support hub.

You will be patient and good humoured. You will have experience supporting and providing services for individuals with learning disabilities, physical disabilities, behaviours that may present as challenging and be able to work in a way that is truly person-centred - aiming to make a real difference to the individual’s lives by promoting independence and enhancing their perception of quality of life. It is important that you understand that this looks different for each individual. You will influence support delivery and role model positive service culture.

You will lead support directly for 2 individuals living within the scheme.
Melissa has a diagnosis of Profound Multiple Learning Disability and requires full support around her college attendance. Being comfortable with personal care, cuddles on the floor and cartons is a must.

Kerry has epilepsy and can display signs of anxieties at times of feeling overwhelmed. Kerry loves to look around charity shops, knit and get out for nice lunches but really enjoys the comforts of home.

For the right candidate this role may also include the opportunity to work with the other 5 individuals who also live in the service on either 1:1 support or core background support from our onsite support Hub.

The Role

The Role

As a Support Worker, you will apply person-centred support practices to enrich Colleen’s life.

Specifically, you will guide her to achieve her goals as she seeks meaningful day opportunities; by creating environments, choices and opportunities that enable her to fulfil her potential.

Meeting her changing needs, you will follow and contribute to support plan guidelines that both reflect and help to manage Colleen’s health and personal needs.

You will engage in non-judgemental positive-behaviour support with an outcome-focused approach, whilst intervening in distressed behaviours in the least restrictive manner possible.

Additionally, you will:

- Identify and encourage learning opportunities for the person you support
- Engage with external partners including medical professionals and social workers
- Accurately complete all records such as incident reports and support records
- Engage in debriefs after any incident or error (practice a culture of learning)

About You

To be considered as a Support Worker, you will need:

- Experience supporting people with learning disabilities and autism
- Knowledge of positive behaviour support
- Experience of safeguarding people
- Care certificate and National Diploma level 2 or a commitment to work to this
- A full, valid driving licence and access to a vehicle

Sarah is a cheerful, motivated and chatty lady who enjoys being busy – it is important she has her days structured and knows what she is doing. Sarah usually attends a local day centre but has been unable to due to the Covid restrictions, we are seeking to enhance her opportunities to socialise and enjoy life in general. Sarah needs to be supported to make suitable choices and decisions but is a lady who knows her own mind and what she wishes to do and achieve each day. Sarah needs support with managing her finances, medication, health needs and some personal care - the support team work closely with Sarah to develop skills and confidence, promote independence, and to lead a full life in the local community.

Sarah has always enjoyed walking and getting out and about and will appreciate the opportunity to have a coffee and a chat to anyone happy to pass the time of day – although she doesn’t like the rain (or spiders!). Sarah has recently begun swimming each week and would like staff who are confident to continue to support her to achieve this. Sarah has also recently started going to watch rugby and is really enjoying the opportunity to broaden her horizons and explore other activities that she might enjoy.

Sarah has epilepsy, a learning disability, and a rare genetic condition; the team are supported by specialists to ensure her support is adapted in line with her changing needs. Sarah would like to find new members for her team who will have a positive impact on her life by providing her with consistent daily support, understanding the need for routine, structure and predictability to her week and provide emotional support when needed.

It is very important to Eddie that he is supported to make choices and decisions that are important to him (recognising too many choices can cause him to feel anxious and overwhelmed) and that his staff team promote his independence, community inclusion and active citizenship. It is important to Eddie that other people ‘stick to the rules’ and do not make changes to plans they have agreed to. Eddie likes staff who will ‘get stuck in’ and join in on some of his activities, this includes going to the gym, and swimming, taking part in shallal and other social/ creative activities, landscape and portrait painting and going for long walks both locally and further afield.

Matt is currently in hospital in Cardiff and he is moving to Brixham. Matthew has his own home to move into in Brixham and is looking for a team that can support him transitioning him from hospital to moving into his own home. As part of the transition, there is a 4-week intensive induction plan – which involves training & travelling to Cardiff to meet the individual (expenses paid) and working closely with the family local to Brixham.

Here are some words from Matthew’s family

“We would like Matt’s support staff to learn as much information about Matt from what we have submitted, existing carers and their own observations of Matt interacting with those around him. Knowledge of Matt, his behaviours (anxious and quiet moments) and interests is vital for all staying in his home. Matt has trouble understanding the do's and don'ts of life, but he does have a high level of intelligence. Reading, writing, excellent recall, operating a laptop, iPad etc and games he does enjoy pen and paper hangman. Patience and flexibility with Matt, there are numerous interests he finds enjoyment in. Music, dance, stories, film scenes called up from the internet, films and comedy series. We feel it's important to indulge him when wanted.
We are expressive when talking to Matt about some of the above and Matt does mimic some of the characters. Support will need to be attentive to Matt's needs i.e. hygiene, teeth, hair and shaving, exercise with outings, correct attire for the elements, hot water and room temperature.”

The start date for this role is due to commence on the 25th April 2022.
